
My name is Jarod R. Martin and I work as a case manager through Jodi’s Heart Inc. I have been working as a case manager since June of 2008. I am certified in all three areas of case management: Developmentally Disabled Children, Adults, and Adults with an Acquired Brain Injury. While I have only been in this field since 2008, I did grow up with a sister with Down’s Syndrome and have a lifetime of experience working with the special needs population. As a case manager I am responsible for overseeing the care of all my clients by all their providers. I ensure my clients are receiving the services they need, I help to connect them with any services they want, and ensure that if they have any unmet needs, I am helping them satisfy those needs. I work with both the clients and their guardians (if they have one) to ensure my clients are truly receiving the best services for them and to ensure their services are truly participant-driven.
In addition to regular case management services, I also perform Targeted Case Management services. Targeted Case Management is for clients who are looking at getting funding through the State of Wyoming Medicaid Waiver. With this service I will help clients and/or their guardians through the application process to help them get funding. This will allow them to receive the necessary services to allow them to live as independently and safely as possible.
